Richfield Living, in Salem, VA, is accepting applications for a creative, motivated and dynamic Clinical Coordinator RN/LPN to work with our long-term care team. This is a FT/Exempt team member position. The position will transfer to Roanoke when new campus is open.
Richfield Recovery and Care Center (soon to be Richfield Health Center) has an opening for a Clinical Coordinator. Richfield is working to transition the long term care institutional culture into a household living community, and we are looking for an energetic, dedicated RN/LPN to join our Nursing Management team on this journey. The current opening is on a 50 bed long term care unit, which is divide between to Clinical Coordinators that which will transition into a 19 bed household. The ideal candidate is an RN/LPN with 3-5 years of long term care supervisory/management experience with knowledge of state and federal regulations. MDS experience preferred. Must possess excellent communication and customer service skills. Come join us on our journey towards Household living and Resident Directed care!
Responsibilities:
- Follow facility policies and procedures to implement medication and treatment order.
- Ensure household and resident directed care is promoted and followed.
- Completion of MDS for household
- Assist with staffing and scheduling for household.
- Assure residents' comprehensive care plan is followed by nursing team on their assigned unit.
- Promptly report changes in residents' conditions to physicians, supervisor and responsible party, and take follow up action as necessary including providing immediate emergency care as needed.
- Follow established standards of nursing practices and implement facility policies and procedures.
- Supervise and evaluate direct resident care and initiate corrective action as necessary.
- Must have a current RN/LPN license that is valid in Virginia and be in good standing.
- Must possess long-term care leadership and management experience
Qualifications:
- Licensed RN/LPN in the state of VA
- 3-5 years management in a long-term care setting
- Memory Care experience required
- Acute care experience preferred
- Possess excellent oral and written communication and computer skills
- Extensive knowledge of state and federal regulations and state survey process required
